Fall 2008 Late Registration

Late Registration will be accepted by mail until 6/30/2008.  After 6/30/2008 applications and payment will be received and the player(s) will be placed on a waiting list.  Names will be taken off the list if needed in order or date received and players rating.  Please do not ask since there will be no requests granted for teams, coaches, or teammates.

Procedures for submitting a late application are as follows:

1. Go to eayso.org.  Pre-register your player(s)
2. Print two copies of the form and sign both.
3. Include payment

• $105.00 per child, $95 per sibling (after May 30, $145 per child if space is available) Made out to FC AYSO
• $25 Non-Resident Fee (per child)…Required by the City of Foster City for players who DO NOT live in Foster City made payable to Foster City Park & Rec.
